July 18, 1993:
13:17. 911 was called for a suspected foul play with a patient of Pinevalley Hospital Center.
13:19. Officers Luis Montee and Ibrahim Trussell arrived at the scene alongside crime scene investigator Camilla Mattera.
13:22. Officer Trussell secured the room with crime scene tape while Montee took the doctors, nurses, patients, and family members who were close by throughout the past few days.
13:22 Mattera scanned the room and searched for potential evidence marking them as she went.
13:23 Trussell talked to the pathologists about the patient's body.
Notes taken: The patient was a 28-year-old Middle Eastern man named Zaynab Samir. The autopsy report indicated he had Amoxicillin in his system which caused his anaphylaxis shock. His medical records did indicate an allergy to Amoxicillin which makes this a potential homicide.
13:24 Mattera welcomed the photography team and led them to the patient's room. Photos were documented. A crime scene sketch was also made.
13:33 Mattera proceeded to secure and collect the evidence by carefully and with gloves taking samples of the bag of supposed IV fluid, the bloody needle, the bottle of Amoxicillin on the table, the pieces of hair on the floor, a few hard-to-see fingerprints on the bottle of Amoxicillin, and asked the hospital for the security camera footage, particularly the one in the hall outside the room. Mattera handed the evidence to the forensics analysis team who documented their names with the evidence.
13:35 Montee gathered the staff for questioning. Head Doctor Blakely Woody gave the staff schedules and logs of the day.
The list of people who entered the room was recorded, and the people were gathered. Clifton Bolan, Ashton Almeda, and Robert Loiacono were the only doctors to enter the room. Zaynab Samir's mother was the only other person to enter the room. Clifton Bolan reported the unusual death.
13:45 Montee and Trussell gathered the suspects and returned them to the station for questioning. All suspects were willing to go. Ms Samir was bawling her eyes out, unable to get much of a word out. So she was brought to a counselor and will be questioned later.
The following are the transcripts from each suspect's questioning.
T: What were you doing in Mr Samir's room today?
B: I was there to check on him after Loiacono went to give him some medicine. Though he said he didn't give him anything.
T: And did you see anything? We saw on the camera that you walked outside the room a few seconds before Loiacono left. Was there anything suspicious going on? Is it typical to not give them any medicine at this hour?
B: Not that I can recall sorry. I saw Loiacono give Mr Samir some water, and he told me he was feeling ill so he didn't give him anything. He said Samir was getting hot and didn't want to take any risks. You know how newbies are. Don't want to hurt them with pain meds.
T: And you told us at the scene that Mr Samir ripped the IV out of his arm complaining of the pain and itchiness coming from there correct?
B: Yes, which is also when I noticed in his notes he's allergic to Amoxicillin which a container was sitting on the table.
T: So do you say He was killed by Amoxicillin?
B: I would presume so yes.
T: thank you for your time today, please feel free to reach out with anything else. But for now, I'll ask if you'd go have a statement with Mr Montee.
T: So Mr Loiacono you're new to working here I see.
L: Yes I am.
T: We were told that you refused to give Mr Samir anything. Is that correct?
L: Yes that's true I didn't know if I should've given him anything he was burning hot and it could have been from anything so I asked Mr Bolan to check him out for me. I was scared because he was just a day away from being discharged. He had no reason to be getting that hot. He had A temperature Of 39°C.
T: And did you read the patient's notes beforehand?
L: Yes I make sure I always do.
T: And did you see anything important?
L: Yes he had quite a few allergies so I was scared that I could give him something that would make him feel worse, and no matter how much I double-checked the pain meds I just couldn't get myself to give it to him.
T: And who was with Mr Samir before you, do you know?
L: Yes I believe Ms Samir was there first thing in the morning with me then she left to run errands and Almeda said she could take over for me.
T: And do you know why she wanted to take over? We saw it wasn't part of the schedule.
L: No but I just assumed it was a quick switch or she wanted to give me a break, you know cause I'm new and all.
T: thank you for your time if you think of anything else please let me know. But for now, I'll ask if you could go take a statement with Mr Montee.
T: Hello Almeda. So we heard you went to Mr Samir's room earlier today even though you weren't scheduled there. Wanna tell us why?
A: Yeah, um I wanted to make sure everything was going well for Loiacono.
T: Ah because he's new?
A: Yes! Mhm
T: So do you happen to know Mr Samir at all?
A: No, not at all.
T: Okay, well I'll ask you to please wait here. I have to talk to my colleague.
A: Mhm
13:45 The forensics team brought the evidence to the lab.
July 25, 1993:
10:00 The forensics team reported back with an analysis of the liquid in the IV, the blood on the needle, the fingerprints on the bottle of Amoxicillin, and the hair on the floor.
The team reported. Amoxicillin was found in both the blood and the IV bag. They analyzed the fingerprints and determined the ridges were closest to Almeda. The hair sample also was closest to Almeda but without the follicle holding the DNA it is just class evidence.
11:32 Almeda was interviewed again. With questions of if she knew Me Samir was allergic to Amoxicillin and if she gave it to him.
11:45 The interview went sideways and Almeda was arrested for trying to run away.
12:00 Ms Samir was able to give information that her son and Dr Almeda had previously dated and it didn't end well.
12:05 Almeda was put in a jail cell for further questioning and trial.
